PELVIC FLOOR PHYSICAL THERAPIST

Dr. Rebekah Slafka, PT, DPT, PRPC

I’m passionate about all things pelvic floor—and providing care that helps patients feel seen, heard, loved, and empowered. 

I have been working in this specialty since 2018, with expertise in helping patients—women, men and those within the LGBTQIA+ population—with diverse diagnoses. I am one of the few physical therapists in Ohio to earn a PRPC accreditation from the international Herman & Wallace Pelvic Rehabilitation Institute, and I’m also a certified Endometriosis Physical Therapist on iCareBetter.

I am manually trained and believe in taking a biopsychosocial approach to care. My specialties include cupping, integrative dry needling, muscle energy techniques, visceral mobilizations, myofascial release, and more. 

Beyond my skillset, I’ve also experienced living with pelvic floor dysfunction and chronic pain. I know firsthand how much pelvic floor dysfunction can affect your life—and how hard it can be to find a provider who understands and advocates for you. My goal is to be both your provider and your partner in achieving sustainable, functional wellness.  

When I’m not doing all-things-pelvic-floor, you’ll find me enjoying our hobby farm, gardening, reading, and spending time with my husband and three daughters.

PELVIC FLOOR PHYSICAL THERAPIST & ORTHOPEDIC SPECIALIST


I grew up in India, and received my B.S. and M.S. of physical therapy from the University of Mumbai. I also taught at the undergrad and post-grad physical therapy program at Manipal Academy of Higher Education, which is a very reputable private institution in India. In 2015, I obtained my doctorate in physical therapy from A.T. Still University in Arizona, and have been an American PT Board-Certified Orthopedic Clinical Specialist since 2021. I’m also certified in IASTM, proficient in manual therapy, and have my Pelvic Floor Level 1 certification.

Having treated patients of all ages, both abroad and across the U.S., I’ve had the pleasure of interacting with people from all different backgrounds and outlooks. It’s very important to me to bring that cultural awareness to my practice, creating a space where every patient can feel comfortable, safe, and heard. 

I love keeping up with medical advances in our field and how we can continue to improve the care we offer patients. In addition to publishing research articles, I serve as an Honorary Research Fellow at the Mahatma Gandhi Institute for Health Sciences, mentoring physical PhD students as they study the biomechanics of Indian classical dance and injury prevention. As a trained Indian classical dancer myself, it has been really fun to combine these two worlds!

At home, I have a great husband, two lovely college-going kids, and an adorable dog. I enjoy spending time with my family, traveling and learning about new places and cultures, trying out different foods, dancing, and staying fit. I’m passionate about all things pelvic floor—and providing care that helps patients feel seen, heard, loved, and empowered.

Dr. Deepa Kalliath, PT, DPT, OCS

Hearing a patient tell me they are feeling better or experiencing an improved quality of life after working together—that is absolutely my favorite part of what I do. I’ve been a physical therapist for more than 25 years, and the work I do never gets old: I truly love it. I’ve also experienced firsthand the difference that pelvic floor physical therapy can make in your wellbeing, and I want to offer that to as many people as I can.

I believe strongly in taking a treatment approach that is holistic, compassionate, evidence-based, and uniquely tailored for that patient. For me, that means prioritizing patient care above all, and drawing on my diverse experience as a doctor of physical therapy, board-certified orthopedic specialist, and certified yoga practitioner.

Dr. Kat Welzel, PT, DPT

I’ve known I wanted to be a physical therapist since I was in 4th grade. Growing up, I did competitive cheerleading and was fascinated by the impact PT could make on my fellow athletes. People told me I’d probably change my mind—but I never did!

I earned my undergrad degree from Mount St. Joseph University in three years, then stayed to complete my doctorate in physical therapy. In addition, I’ve also earned my Herman & Wallace Pelvic Function Certification and have started to train to sit for my Pelvic Rehabilitation Practitioner Certification .

During my clinical rotations, I found myself drawn to settings that many people did not want to do…like pelvic floor therapy. Pelvic floor PT is intimate and vulnerable work, but it’s where you can have a really big impact on how someone is feeling or functioning. It blows my mind how little attention is paid to it, for men and women, because it’s really so important.

Helping someone achieve a breakthrough, feel relief or regain confidence in their body’s abilities is incredibly meaningful to me. I’m my patients’ biggest cheerleader, and want everyone I see to feel truly safe and supported—especially if past PT experiences left them feeling unheard or dismissed.

I first met Becca during an elective class she taught at MSJ, and a few weeks later, I had the chance to shadow her at Restore. At the time, I thought, Working here would be my dream job—and when Becca emailed our program saying she was looking to hire, I replied right away!

I grew up just over the border in Indiana, but I’ve always considered myself a West Sider at heart. Outside of the clinic, you’ll usually find me spending time with my grandparents, my family and my younger brother Logan, or hanging out with my fiancé, who I met in jazz band at MSJ and who is studying to be a respiratory therapist at NKU.

OFFICE MANAGER

Sarah Fisher

When you reach out to Restore, I’ll likely be one of your first points of contact. As the Admin, I keep things running smoothly behind the scenes while ensuring you feel supported from the very beginning.

Although I’m not a physical therapist, I’m deeply passionate about pelvic floor physical therapy. Having lived with chronic pain myself, I know firsthand the difference that skilled pelvic floor care can make—and I can confidently say our therapists are truly exceptional.

Before joining Restore, I served as a director of a pregnancy care center and worked in advocacy roles supporting survivors of domestic violence and sexual assault through both shelter services and the legal system. In each of these roles, I found great joy in walking alongside people during difficult seasons and helping them find hope when it felt out of reach.

When I’m not at work, you’ll usually find me with my husband and our two kids—either curled up with a good book or spending time outdoors together.

Katie Nelson

Hi! I’m Katie, and I’m honored to be one of the first faces or voices you’ll encounter when you reach out to Restore. My hope is that from the moment you arrive, you feel safe, supported, and truly seen.

My background is in victim advocacy, where I supported survivors of violent crime through crisis response and navigating the legal system. I’m currently pursuing my Master’s in Clinical Mental Health Counseling at Antioch University with a focus on trauma. My passion is helping people feel empowered in their healing—especially those whose experiences are often overlooked or misunderstood.

I was drawn to Restore not only for its holistic and inclusive approach to pelvic floor therapy, but because I’ve walked a similar journey myself. I know how vulnerable it can feel to seek help for chronic pain, sexual health concerns, or trauma-related challenges—and I also know how transformative it is to find care that meets you with dignity, knowledge, and compassion.

Outside of work, I’m a creative spirit. I run a photography business, write poetry, and love spending downtime drawing, painting, reading, or recharging at home with my partner and our three quirky cats. I’m also a lifelong advocate for equity, affirmation, and belonging—in every area of life, including here at Restore.
